From a tramway perspective, Melbourne is also a "region". Once you get
away from Melbourne 3000, trams operate to many suburbs, no doubt
including some other "cities".
In Sydney, the CSET will run from City of Sydney to the City of
Randwick. No doubt Adelaide has similar arrangements.

Perhaps Brisbane was the only large system to be contained within a
single city boundary?

There is no suburb called Gold Coast, the same with Hervey
Bay. People usually say they live on THE Gold Coast which covers all
areas between the NSW border (which actually runs east west down the
middle of the Main Street of Coolangatta), the mountain range several
Km to the west and somewhere near Ormeau in the north.
