Do they leave the orange bars to be entombed into the concrete or do they remove them as the concrete is poured ?

The Sydney Pandrol Q-Track construction method has the jigs above the rail and they un-thread the bolts and reuse the jigs. The track gauge is held by the cured concrete only after the jig is removed. The holes left by the supporting threaded rods are presumably filled with slurry.

According to the Pantrol web site, the Q-Track rails can be removed by marking a saw cut down each side and yanking the rails out of the slot. The replacement rails are then put in the slot and secured with a poured compound - similar to the way the original SLR street track was constructed in the 1990s. While the material is curing the rails are held in position with a jig - probably the SAME jig being currently used to hold the rails now when doing the final layer on concrete.

In George street where the tracks cross the chambers they built under the tracks (presumably for the APS switch gear, there are two of these chambers per block, under both tracks) there are pre-cast slots for the 6 rails in the roof slab of the switch chamber - presumably the rails will be secured with the poured compound through these slots.
